Birth & Death Certificates
   
The Pima County Office of Vital Records issues certified birth certificates for Arizona births since 1950 and certified death certificates for deaths which occur in Pima County.

Birth and Death Certificates
Certificate Fees:
We accept cash presented in person (not through the mail), debit cards, Mastercard, Visa, Cashier's Checks, and Money Orders

Birth Certificates:
     $15 Electronic Certified
     $25 Corrections/changes
Death Certificates:
     $15 Certified
     $25 Corrections/changes


When you come in, take a number and fill out an application form available in the lobby . You may wait for a couple of hours. Please plan accordingly.

Requirements to Obtain a Certified Birth Certificate:
NOTE: Laminated Arizona Driver's Licenses or laminated Arizona State ID's cannot be accepted.



Persons Born Before 1950

Pima County Office of Vital Records can only issue Certified Birth Certificates for persons born 1950 through present.


If you were born before 1950, you must apply to State Office of Vital Records in Phoenix for a Certified Birth Certificate. If you have internet access, you can send for a Certified Birth Certificate at VitalCheck. There you can order online or by fax and charge the certificate to your credit card. To download applications, go to the Arizona State Department of Health Services.



Persons Born After 1950

Pima County Office of Vital Records can only issue Certified Birth Certificates for persons born in Arizona in 1950 or later.
   

bullet Requirements to Obtain a Certified Birth Certificate:
NOTE: Laminated Arizona Driver's Licenses or laminated Arizona State ID's cannot be accepted.

For yourself:
1. A valid government issued photo identification, showing your signature.

Parent of Person:
1. A valid government issued photo identification, showing your signature.

Note: Adoptive parent(s) name must appear on the birth certificate.
Certified Birth Certificates for new born infants are available approximately 7 to 10 working days after delivery date.

Brother/Sister of Person:
1. A valid government issued photo identification, showing your signature.
2. Your own certified birth certificate showing parent's name for verification.

Son/Daughter of Person:
1. A valid government issued photo identification, showing your signature.
2. Your own certified birth certificate to provide relationship

Grandparent of Person:
1. A valid government issued photo identification, showing your signature.
2. A certified birth certificate of your son/daughter
3. Your son/daughter must be named as parent on your grandchild's birth records

Note: A parent must apply for a minor child

Legal Guardian of Person:
1. A valid government issued photo identification, showing your signature.
2. A certified court order of guardianship showing legal custody
3. Paternal legal custody must be established on the birth certificate

County, State or Federal Employee Identification

Note: Expired, cracked, broken, taped or faded identification can not be accepted



bullet Requirements to Obtain a Certified Death Certificate:

Pima County Office of Vital Records can only issue duplicate death certificates for the FIRST 30 DAYS following the registration of the event. AFTER 30 DAYS, death certificates must be obtained from the State of Arizona Office of Vital Records.


Requirements to Obtain a Certified Death Certificate:

Parent of Person
1. A valid government issued photo identification
Note: A parent must apply for a minor child. Adoptive parent(s) name must appear on the record.

Brother/Sister of Person
1. A valid government issued photo identification
2. Your own certified birth certificate showing partent's name for verification

Son/Daughter of Person 
1. A valid givernment issued photo identification
2. Your own certified birth certificate to prove relationship

Note: A parent must apply for a minor child

Grandparent of Person
1. A valid government issued photo identification
2. A certified birth certificate of your son/daughter
3. Your son/daughter must be named as parent on your grandchild's birth records

Legal Guardian of Person 
1. A valid government issued photo identification
2. A certified court order of guardianship showing legal custody
3. Paternal legal custody must be established in birth records

Note: Minute Entries or Issuance Letters will not be accepted
