 Letter to Schools: Varicella Reporting Notice (July 16, 2008)
The Arizona Department of Health Services and the County Health Departments has developed the Arizona School-based Chickenpox Surveillance System in schools and child care centers in order to monitor (reporting form) the number of cases of chickenpox and the impact of the vaccine in the community. Chickenpox (also known as varicella) has decreased in the last few years since many children are now receiving the vaccination against this disease, and thus it is important to detect these changes.

Requirement for Hib Vaccine Booster is Temporarily Suspended Requirement Suspended
January 3, 2008-August 31, 2008
Due to a nationwide shortage of Hib (haemophilus influenzae type B) vaccine, the Arizona Department of Health Services (ADHS) is temporarily suspending the requirement for children in child care settings to receive a Hib vaccine booster dose after 12 months of age.
4-Day Grace Period for Vaccine Doses Administered in Arizona
The Advisory Committee on Immunization Practices (ACIP) allows for most vaccine doses administered within 4 days of the recommended minimum age or interval to be counted as valid. However, ACIP does not allow a 4-day grace period between doses of varicella and MMR vaccines. They must be administered on the same day, or at least 28 days apart.

New Vaccine Rules for Arizona as of January 5, 2008 -
A vaccine rule amendment adding Tdap and Meningococcal vaccine to school immunization requirements, and lowering the age of the Hepatitis A requirement in Maricopa County child care settings, has been approved. Request for Exemption to Immunization

If you wish for your child to be exempt from the immunization requirements, this form must be completed, signed and returned to the school. By state law, (A.R.S.' 15-873) your child will not be allowed to attend school until either a record of immunization or this exemption statement is submitted. for Persons Aged 4 Months–18 Years Who Start Late or Who Are More Than 1 Month Behind | Español - This table provides catch-up schedules and minimum intervals between doses for children whose vaccinations have been delayed. A vaccine series does not need to be restarted, regardless of the time that has elapsed between doses. Use the section appropriate for the child’s age.
