Getting Started with WIC & FOOD Plus |

WIC & FOOD Plus services are available by appointment or on a walk-in basis Tuesday through Friday from 8 A.M. to 5 P.M. and Saturday from 8 A.M. to 1 P.M.

To find out if you are eligible for WIC & FOOD Plus services you will need to bring:
 Each family member you want to enroll:
 who are pregnant or post-partum women less than one (1) year
children up to six (6) years old
Seniors over 60
Identification for each family member you want to enroll. For example:
Birth Certificate
Driver’s License
Immunization Record
Military Identification
School Identification
Voter Registration Card
Proof of current income for everyone living with you. For example:
Pay stubs showing wages
AHCCCS award letter
DES (Food Stamps, TANF) award letter
SSI statements
Verification of other public assistance
Any document showing all money coming into your household
Proof of address. For example:
Current utility or cable bill
Rent or mortgage receipt
Driver’s license
Current piece of business mail
Pay stub with current address
Letter from home owner who you live with
Proof of custody, guardianship or other legal documents if applying for children who are not yours
Immunization records for children under two (2)
You may be eligible for WIC or FOOD Plus services if you’re annual income in less than:

Como Empezar: WIC y FOOD Plus |

&FOOD Plus son disponibles for cita o de Martes a Viernes de 8 A.M. a 5 P.M. y el Sábado de 8 A.M. a 1 P.M.
Para saber si es elegible para WIC o FOOD Plus necesita traer:
Cada miembro de la familia que desee registrar:
Mujeres embarazadas o que han dado luz en el último año
Niños hasta seis (6) años de edad
Adultos mayores de 60 años
Identificación para cada miembro de la familia que desee registrar. Por ejemplo:
Acta de nacimiento
Identificación escolar
Identificación militar
Licencia de manejar
Registro de vacunas
Tarjeta de registro para votar
Comprobante de ingresos de todas las personas que viven en su hogar. Por ejemplo:
Talones de cheque de los últimos dos meses
Letra de aprobación para AHCCCS
Letra de aprobación de DES (Estampillas de Comida, TANF)
Verificación que recibe Ingresos del Seguro Social
Verificación de asistencia publica (Section 8)
Letra de asistencia financiera para estudiantes
Prueba de otros ingresos que estén recibiendo en el hogar
Prueba de domicilio. Por ejemplo:
Recibo de renta o hipoteca
Recibo actual de servicios públicos o de cable
Licencia de manejar
Letra del dueño de la casa en que usted vive
Talón de cheque con su dirección actual
Pieza de correspondencia actual de algún negocio
Prueba de custodia o otros documentos legales si esta aplicando para niños que no son de usted
Registro de vacunas para cada niño(a) menor de dos (2) años de edad
Usted Puede ser eligible para WIC o FOOD Plus si el total de ingresos en su hogar es menos de:
