Immunization Information & Requirements
- IDR and Exemption forms for Schools and Childcare Centers

- Arizona schools must submit an annual Immunization Data Report (IDR) on students enrolled in preschool, pre-k, kindergarten, sixth and tenth grades by November 15th each year. Childcare centers, small group childcare homes and Head Start programs must submit annual Immunization Data Reports on children born on or after 10/1/2003.
